Sagittal SE T1-weighted image shows destruction of the L4 and L5 vertebrae, with collapse of L4-L5 intervertebral disc space. Posterior part of S1 upper end-plate is also partially collapsed, and a grade I retrolishesis is associated at this level. A prominent heterogeneous hypointense mass (abscess) is observed along the anterior surfaces of the lumbar and sacral vertebrae, also a prominent subligamentous epidural component compressing neural elements is present along the posterior surfaces of the L4 to S1 vertebrae.
 
Sagittal FSE T2-weighted image shows destruction of the L4 and L5 vertebrae, with collapse of L4-L5 intervertebral disc space. Posterior part of S1 upper end-plate is also partially collapsed, and a grade I retrolishesis is associated at this level together with L5-S1 intervertebral disc destruction. A prominent heterogeneous intermediate to hyperintense mass is observed along the anterior surfaces of the lumbar and sacral vertebrae, also a prominent subligamentous epidural component compressing neural elements is present along the posterior surfaces of the L4 to S1 vertebrae.
 
Sagittal post-gadolinium SE T1-weighted image shows intense contrast enhancement of the epidural abscess component, and peripheral contrast enhancement of the anterior paravertebral ascess. Involved vertebrae and adjacent intervertebral discs also heterogeneously enhance with contrast material.
